Application Procedure for Certificate of Criminal Record

("Toko Shomei" or "Hanzai Keireki Shomeisho")

We would be grateful if you could please proceed with the next steps.

1 Persons who can apply

In principle, those who have been required to submit the certificate by foreign official organization and you must be either of the following to apply:

• Registerd as a resident of Yamanashi Prefecture
• Former-resident residing in foreign country whose last registration place in Japan was Yamanashi Prefecture

Fingerprints will be taken at the time of application, so please be sure to come in person.

4 Necessary Documents

Registered residents in Yamanashi Prefecture

～Japanese Nationals～

(1) Your own passport
(2) Items that can confirm your name and place of residence

• Certificate of residence (issued within the last 6 months without Individual Number)
• Individual Number Card (as nicknamed “My Number Card”) 
• Driver's License

(3) Documents that can confirm the need for the certificate to be issued _*1

～Foreign Nationals～

(1) Your own passport
(2) Items that can confirm your name and place of residence

• Certificate of residence (issued within the last 6 months without Individual Number)
• Residence card
• Special Permanent Resident Certificate
• Driver's license and so forth

(3) Documents that can confirm the need for the certificate to be issued _*1

Those who reside in foreign countries and whose last resident registration in Japan (alien registration) was Yamanashi Prefecture _*2

～Japanese Nationals～

(1) Your own passport
(2) Items that can confirm your current address in a foreign country (current driver’s license or postal items addressed to you)
(3) Removal of residential certificate or supplementary family register (issued within the last 6 months)
(4) Documents that can confirm the need for the certificate to be issued _*1

～Foreign Nationals～

(1) Your own passport
(2) Items that can confirm your current address in a foreign country (current driver’s license or postal items addressed to you)
(3) Documents issued by a public agency confirming that you have resided in Yamanashi Prefecture (Removal of residential certificate)
(4) Documents that can confirm the need for the certificate to be issued _*1

*1 Examples of documents that can confirm the need to issue the certificate

Please inquire in advance as it depends on the country and reason for the request.

*2 Application procedures for issuance can also be made at the Japanese embassy or consulate in the country (region) where you currently reside. For more information, please contact the relevant local agency directly. In addition, if the application is made at an embassy, please note that you cannot make the same application twice in Japan.

5 Handling Fee (we don't accept paying by cash)

500-yen in Yamanashi Prefectural shunyu-shoshi (Yamanashi income certificate stamp)
Please purchase the stamps beforehand at the police station or at Yamanashi Chuo Bank (where certificate stamps are sold) before coming to our office.

6 Issuance of the Certificate 

• It takes about 10 days from application to issuance. We do recommend that you will apply it well in advance.
• You must come to the office in person to receive the certificate.(Hand delivery is NOT available.)
• If you are unable to receive the certificate in person for some unavoidable reasons, a power of attorney can be used to accept it on your behalf. We will give you a power of attorney.

7 Referents

• Depending on the country to which the application is submitted and the reason for issuance, it may not be possible to immediately accept the application, so please call the above-mentioned Identification Division in advance.
• Certificates will be issued in five languages (Japanese, English, French, Spanish, German) and in a sealed envelope.
